• Receive considerate, respectful, and safe care free from discrimination.
• Be told about your condition, treatment options, and likely outcomes in language you can understand.
• Take part in decisions about your care and refuse treatment to the extent allowed by law.
• Have your privacy and confidentiality protected.
• Have a family member or representative notified of your admission.
• Access your medical records and request an amendment.
• Be free from restraints or seclusion that are not medically necessary.
• Voice a concern without fear of retaliation, and have it reviewed promptly.

• Share complete and accurate information about your health and history.
• Tell us if you do not understand your plan of care or cannot follow it.
• Treat staff and other patients with courtesy and respect.
• Follow our facility rules, including those about visitors and smoking.
• Meet your financial obligations or ask about financial assistance.
